Dress as though you will be interviewed because there is always the chance that some HR person will just decide to interview after completing the application.

There is no reason not to dress to impress.

Have you had the interview yet? I would definitely dress as if you were going to one. You never know if they might actually conduct an interview or you might even meet the nurse manager who is interested in hiring you! Always dress to impress, and even when you get the job, still dress to impress. At my hospital, it is a policy to either come to work wearing business casual attire OR scrubs if you work in a clinical area, but never street clothes.
